COVID 19 Testing Programme Consent Form

This COVID 19 testing programme is being led by the Department for Health and Social Care and the Scottish Government to provide asymptomatic testing in Colleges and HEIs. This consent form has been mandated by government, and must be completed by anyone wishing to obtain tests from the University.

Taking part in testing is voluntary. There is no expectation or obligation to participate. Nobody is required to undergo testing without consent, and nobody will be excluded from campus if they do not wish to test.

Declaration:

I have had the opportunity to consider the information provided to me by the University about this testing programme. I have had the opportunity to ask any questions about the programme and, if I have, I have had these answered satisfactorily.

By submitting this form you are agreeing to the following:

1. I consent to participate in this testing programme.

2. I have understood that my data will be held and shared in accordance with the data privacy notice.

3. I agree that if my test results are confirmed to be positive, I will inform the University to support contact tracing and arrange a follow up PCR test within 2 days to confirm if I have COVID-19.

4. I agree to accurately record all of my test results using the online portal (this can be accessed via www.covidtest.scot) or by phoning 119.
